Our Case Management services provide personalized support to individuals and families, helping them navigate complex health and social systems. We focus on advocacy, resource coordination, and tailored care plans to enhance the well-being and independence of our clients.

Family Residential Services, previously known as Subcontracting Services, have been providing support since 1991. We offer 245D services for adult family and corporate foster care providers, who must hold an Adult Foster Care (AFC) license. Each provider collaborates closely with a Thomas Allen Designated Coordinator to ensure personalized, person-centered care for every resident in a Family Residential Services setting.

Community Residential Services offer 24-hour support in community-based homes for individuals with disabilities. Our team provides personalized care, fostering independence and community integration. Each resident receives tailored services to meet their unique needs, promoting a fulfilling and inclusive living environment.

Community Living Services includes 24 Hour Emergency Assistance, Individualized Home Supports (IHS)  or Supported Living Services (SILS) enhancing adaptive skills, household management, community participation, and health, safety or wellness. Many services are provided in person, or live, remote services are available. Our goal is to help individuals thrive in their communities with the right level of support.

Respite Care services provide temporary relief for primary caregivers by offering professional, compassionate care for individuals with disabilities. Our respite care ensures that loved ones receive the support they need in a safe and nurturing environment, giving caregivers the opportunity to rest and recharge.

Managed Care Coordination ensures clients receive comprehensive, individualized healthcare plans. Our team works collaboratively to coordinate services, streamline care, and improve health outcomes, providing the support needed to navigate the complex healthcare system effectively.
